School to Community Transition Job Coach

The LARC School to Community Transition Program works with students 18-21 years old with ASD, emotional, and/ or intellectual challenges. The School to Community Transition Job Coach will support academic, social, behavioral and vocational initiatives by assisting in job training, community involvement, and daily living skills. The successful candidate must be able to encourage students to work to the best of their abilities and to take pride in their achievements, providing a stimulating environment in which students learn, interact and grow academically, socially and emotionally.

Essentials Duties and Responsibilities include, but are not limited to:
  • Assumes responsibility for helping carry out daily program goals, designed in accordance with each student’s Individual Education Plan (IEP)
  • Provide input for assessments designed to monitor students’ progress in attaining his/hers goals.
  • Assist in implementing daily scheduled activities, and helping transition between each activity to be as expedient and as smooth as possible.
  • Assumes responsibility for supervision of assigned individuals during the day.
  • Follow-through with all designed behavior management programs, activities of daily living programs developed to increase individual’s abilities.
  • Assist in compiling daily summary reports, including collecting and recording data on each goal or program if/as assigned
  • Assist individuals with tending to personal needs and mobility as needed.
  • Assume driving responsibilities
Qualifications/Requirements:
  • 2-5 years related experience with working with young adults with autism and other developmental disabilities as well as challenging behaviors.
  • CPR, PMT, First Aid Certifications required.
  • High School Diploma or General Education Degree required; proof will be requested.
  • Valid CT driver’s license required; proof will be requested. Public Service License; plus
  • Ability to lift and/or move up to 50 lbs.
