WHY YOU SHOULD BECOME A DIRECT SUPPORT PROFESSIONAL
Responsible for coordinating program plans for individuals in LARC’s day program. Disseminating of program planning information to staff and external service providers as appropriate and for oversight of the ICF recording/reporting systems and DDS regulations. Supports LARC’s mission, vision and guiding principles.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Coordinates day program planning. Coordinates the development of written monthly, quarterly, sixth month and annual reports and distributes those reports to appropriate external service providers, LARC staff with copies to the main clinical file.
The QIDP attends quarterly, sixth month and annual individual planning (IP) meetings as well as special interdisciplinary team meetings (IDT) as determined by the Clinical Support Coordinator and DDS invitations. Reviews monthly behavioral data of participants, shares patterns of concern with the clinical staff, program coordinators and support team members. Provides written progress reports on identified goals based on current data, assessments and staff input.
Actively advocates for the individual’s needs as identified through the planning process and interviews with the individual. Provides and circulates written post meeting notes to internal teams which identify areas of concern and new information as well as any further goal changes. Provides documented written follow up on any missing external team reports to the DDS case manager/ DDS case manager supervisor in order to obtain timely and accurate individual plans, levels of need and water safety information for LARC files and for inspections.
Ensures that residents of ICF facilities receive active treatment in their vocational program settings through the following activities:
Works as an active member of appropriate teams as necessary.

Education and/or Experience
Minimum of a Bachelor’s degree in human services or related field, with 2 years of experience in intellectual disabilities, including a knowledge of interdisciplinary approaches to program planning, intermediate care facilities standards and requirements and coordination of program planning systems. Clinical education and/or experience preferred.
Certificates, Licenses, Registrations
CPR, First Aid Certification, PMT, Valid Connecticut Driver’s License

- Oversees the ICF recording/ reporting systems in order to ensure compliance with ICF regulations.
- Coordinates with program coordinators, meetings with Health Department and IPR-UR Surveyors for annual re-surveys or more frequently if necessary.
- Facilitates the survey process by accessing records, providing information and coordinating other staff as necessary to meet with surveyors.
- Follows through with individuals or departments during surveys regarding any missing or incomplete components in an effort to minimize citations.
- Attends and participates in all exit interviews following all surveys.
- Assists vocational administrators in timely development of agency plans of correction.
- Assists individuals and departments in planning/implementation of system changes as necessary to correct deficiencies.
- Assists vocational administrators to coordinate and ensure immediate correction of ICF/MR and IPR-UR standards cited and help to prepare documentation necessary for return to designated monitoring agencies.
- Monitors progress toward deficiency correction through audits and follow up and provide written status reports to vocational administrators and the clinical support administrators.
- Conduct random and periodic checks of corrected areas throughout the year and conduct complete management audit of each participant with ICF status at regular intervals.
